This notice explains what happens to information when you visit www.ChooseJoyMi.com. It covers the website only. How we use and protect your health information as a patient is described in our Notice of Privacy Practices.
Forms you fill out
When you send an appointment request or a referral through our website, what you type goes straight to a secure service that has signed a HIPAA business associate agreement with us. We treat it as protected health information under our Notice of Privacy Practices. We use it only to contact you and set up care. Our office team also receives a copy by email so we can follow up, and the office deletes that email once the request is entered into our records. The copy saved in our secure system is deleted automatically after 90 days, though encrypted backups may keep a deleted record for up to 35 days.
The contents of our forms are never shared with analytics or advertising companies. On form pages, no analytics code loads until your request has been sent.
Regular email and text messages are not secure. Please don’t email us health details. Use the forms or call us instead.
Analytics and advertising measurement (Google Analytics)
We use Google Analytics 4 to understand how people use our site, such as which pages are visited and how many people request an appointment. Google Analytics uses first-party cookies (for example _ga and _ga_<container>) and receives your IP address and general browser information.
We’ve limited what Google receives:
- Clinician profile pages and request pages are reported under generic names, so Google doesn’t learn which clinician you viewed or contacted.
- Search terms, form fields and web addresses with extra details are removed before anything is sent.
- When someone sends a request through our site or taps one of our office phone numbers, we count it as a visit event. That count includes no names, contact details, clinician or reason for the visit.
- Personalized advertising and Google signals are turned off. We don’t use the site for remarketing, and we don’t upload patient or contact lists to Google.
To opt out, you can block cookies in your browser or install the Google Analytics opt-out add-on. The site still works without them.
Hosting, security and site content
Our site is hosted on Amazon Web Services, which processes your IP address and page requests to deliver the site and protect it from attacks and spam. Cloudflare provides our domain name service only: it answers lookups for our domain, while page requests go directly to our hosting. Most pages load clinician and office information from our content provider, Sanity, which receives your IP address when the page loads.
Maps
Our location pages include embedded Google Maps. When a map loads, Google receives your IP address and may set its own cookies under Google’s privacy policy. Our location pages also list each office’s address if you’d rather not use the map.
What we don’t do
We don’t sell or rent your information. We don’t use your information for advertising targeting. We don’t knowingly collect information from children through the website; a parent or guardian should fill out requests for anyone under 18.
